CCHMC Self Screening: Multi Factor Authentication
To complete your daily self-screening assessment, first you need to complete your multifactor authentication, you will have to set up your User ID and password for CCHMC. After this is done, you will be able to complete your COVID screening prior to going to appointments at any locations or even remote jobs.
Click here to view instructions for the multifactor authentication(please follow instructions for pages 7 and 8 ONLY after receive the CCHMC User ID and password). To obtain your user ID and password, you will have to call the CCHMC IT department at (513)636-4100 and have them set up your temporary password. Be ready to have the following information handy prior to your call since they most likely ask for it:
- Full name
- Date of birth
- Last 4 digits of your social security
- CCHMC badge #
Once you have your user name and password, then you can follow the instruction for the multifactor authentication (please follow instructions for pages 7 and 8 ONLY) . After the multifactor authentication is completed, you can go to the following link to complete your COVID screening: covidscreen.cchmc.org. They will ask you for your CCHMC email address and your password (the one that you receive form the IT department). Your CCHMC email is your first name.lastname@cchmc.org
As soon as you log in, you will receive a call on your cell asking you to confirm your login by pressing #. Please note the call will come from this number 855-330-8653. Make sure to answer the call, otherwise, the system will give you an error. After you answer the call and hit the # key. Make sure to go back to the covidscreen.cchmc.org page to go to the questionnaire for you to answer and then it will give you your daily pass. The screening process will have to be followed anytime you have an appointment either remote or on site prior to the start time of the appointment.
